HEALTH AND IMMUNIZATIONS
A health record of each student is kept on file at all times. Parents
should make the school aware of any medical problems a child might have.
Information regarding any chronic illness (asthma, diabetes, etc.), which
might require attention during school hours, should be on file in the
office.
In order for the physical examination to be complete, the following
parent/doctor signature, immunizations, with dates, must appear.
1. DPT (Diphtheria-Pertussis-Tetanus)
2. DT 5 doses
3. OPV (Oral Polio Vaccine ) 4 doses
4. TB tests and results are mandatory as of July 1994
5. Measles given after 15 months – 2 doses; 2nd measles vaccination all
students PK through Grade 8
6. Rubella after one year of age, even if the child has had the disease
7. Mumps
8. Lead Testing mandatory for students entering school for the first time
9. Hepatitis B vaccination required for all children entering 5th grade
and children two years of age or older entering a school operated program
below the kindergarten program.
10. Chicken Pox vaccination mandatory.
No student will be permitted to enter school without the appropriate
immunization.
The Illinois Department of Public Health has required all students (PK
through Grade 8) to have a second Measles vaccination. This regulation
became effective during the 1993-1994 school year. No student will be
allowed in school without this second measles inoculation.
The school will inform parents/guardians of any necessary examinations,
vaccinations, booster shots, or dental care which may be required by
School Law.
