MEDICAL INFORMATION
State law forbids school personnel to administer any type of medication to
students. When a child is on medication for a short term, immediately
following an illness, the parent must determine if it is in the child’s
best interest to keep the student at home during this period of
recuperation.
If medication is necessary to maintain the student in school, parents are
asked to come to the office and fill out the appropriate forms.
NO MEDICATION WILL BE ADMINISTERED TO A
STUDENT WITHOUT PERMISSION FROM THE PARENT.
Cough drops may be brought to school but the parent
must provide a note for the teacher and send to the office.
Physicals with immunizations are mandatory for children entering Grades K
and 5 and for all new students at any grade level transferring into
Epiphany School. Immunizations are kept current for all children entering
Preschool. Lead testing is required for ages 6 and under. Please retain
copies of your health records before sending them to the school office.
