PUBLIC SCHOOL REFERRALS – REQUESTS FOR SPECIAL
SERVICES
The Principal, school counselor, and teacher will determine if a child
should be tested by the Public Schools. The Principal is the liaison
between Epiphany School and the Public Schools. He/she will forward the
forms and, with the Case Manager of the Public School make all subsequent
arrangements for teacher attendance at staffings, any observations,
testing, interview etc. done by the Public School Special Services Staff.
If a parent feels their child is in need of testing or special services
from the public school, he/she should first meet with the teacher, school
counselor and the Principal. If the teacher, school counselor and
Principal agree with the parent, the teacher will get the needed forms
from the Principal and begin the process. If the teacher, school counselor
and Principal do not agree that the child needs testing or special
services, the parent may initiate a request for special services by going
to the public school. They will give the parent the forms, which the
parent will forward to the teacher and the procedure described in the
previous paragraph will be followed.
